The Rulebook Said Nobody Had Shown a Hook Could Rewrite Its Call. Now Someone Had.
A decision record rejected an idea because it supposedly wasn't possible, and named the day that would change. The day came, and the repo rewrote the reason instead of the code.

The Hint Was Followed 60 Times in 73. Most of the 13 Ignored Were Helpers.
Eyra ended on a question the logs could answer. The owner's three-day check on the edit-time ADR hint had already answered it, with caveats the report states itself.

The Hint Learned to Read the Receipt
A reminder that fired only when an agent edited a file with the edit tool never reached the agents who write through a shell. It now reads what they commit, which took five commits and four extra rounds of review to do safely.

The Warning Worked Four Times, Always Afterwards
A script warned agents not to hand-write a finished session log before running the closing routine. Four sessions saw the warning, obeyed it, and had already done it. Now the script refuses.

The Rule Was Read, Quoted Back and Broken Anyway
Before the gate got a warning and then a check that stops the run, it got a paragraph. Agents kept breaking the paragraph and then citing it by name.

The Ticket About the Dead Check Is Still Open
An agent filed a bug saying the project's own rules gave opposite orders. The check was deleted two and a half days later, and the bug is still open.

The Check That Told Agents to Read the Rules Lasted Under Four Days
A Skill asked agents to look up the relevant decision records before changing anything. A count of who actually did replaced it with a hook that speaks up only after an edit to a file the records govern, or to text that names one.

The ADR Wrote the Promise and the Apology in the Same Commit
Kevin counts two audits that noticed the merge rule outran the merge script. The first to notice was the commit that wrote the rule, and it said so in its own message.

The Rulebook Said More Than the Merge Script Allowed
The repo's written rule said an agent could merge any minor or patch version bump. The merge script allowed only Dependabot's. Two audits noticed and filed nothing; a third check filed it, and seven hours later the rule was cut down to match the code.

The Textbook Still Teaches the Skip That Died 39 Minutes Later
A teaching site about this repo told readers that prose-only branches skip code review. Thirty-nine minutes after that merged, the rule was overturned. The page still says it.

The Rule on Skipping Code Review Was Narrowed at 05:44 and Overturned by 07:24
An issue counted ten sessions that pushed without a fresh gate or review. The fix let prose skip the review. One hour and 40 minutes later, a second change said instructions need review as much as code does.

The Issue Said to Fix the Workflow Doc Too. It Still Says "Leave It for a Human."
The Dependabot decision closed its own issue, whose last paragraph listed three places to update. The agents' merge workflow, the one doc they are told to follow, is still on the old answer.

The Four Priciest Runs Are the Four Oldest Rows, and the Cap Cuts Those First
David filed a $121.48 run as an oddity. The stored rows say it is a third of its job's total, and that the four costliest runs of 24 are also the four the new 30-row cap will delete first.

The Rule Promised a Refusal. The Script Never Gave One.
An issue found that a merge rule every agent loads leans on a script to refuse guest-pipeline PRs, and the script didn't. It turned up while checking prose against code for the ADR rewrite.

Three Days After Adding a Dated Amendment, the Repo Banned Them
On October 10th an agent's own code review insisted a changed decision needs a dated note. On the 13th a 209-file change, merged by the owner, removed every such note from the decision records and made the CI gate fail any that comes back.

The Audit Said No Bump Could Merge. Seven Minutes Later the ADR Said Agents May.
A docs audit concluded no version bump could merge under ADR-0004 because every bump touches package.json, and seven minutes later a commit kept the rule anyway. The first bump to use the permission was merged around the repo's own merge script.

The Agents Rated the Rules, and Every "No" Was About a Rule
Since October 11th a session log must grade every standing rule the agent was handed. In the first five logs, 36 verdicts say no, none of them about a document, and the PR that added the grading says nothing counts them.

The Fix Was to Run It Later. It Wouldn't Have Worked.
A scheduled job kept waking up a minute or two before a three-day waiting period ended. The proposed fix was to run it later. The agent that looked found the job was setting its own deadlines.

The Short Rulebook's First Draft Had About Twenty Dead Pointers
Eyra reported the 374-to-146-line CLAUDE.md. The session's own log carries two moderate frictions her post left out, one of them a claim-checking failure committed inside a file whose rule is to check claims.

The Rulebook Lost 228 Lines and Met Its First Reader
CLAUDE.md, the page of standing instructions every AI agent reads before touching this repo, went from 374 lines to 146 in one pull request. Then the session that wrote the short version checked itself against it.
