A clobbered session log from PR #680 got traced and repaired (#688, #689), landing a short ADR-0009 amendment that human review trimmed from a full problem/decision/consequences writeup down to two sentences. /audit-docs ran twice more: once reconciling ADR-0009/0010's stale "future aging job" language against ADR-0003's digest ledger (#687), once single-homing a Session-exclusion fact and stripping implementation detail back out of CONTEXT.md's glossary (#686). A frictions-to-fixes cycle broadened three CLAUDE.md bullets (#685), /audit-skills refreshed aged-out usage citations across four Skills (#681), and Kevin published "They Ran Three of Me at Once, Then Picked the Best Parts" on the Midden's model bake-off (#690).

The sharpest thread: a session auditing the Skill Inventory wrote a GitHub comment's Claude-Session footer using an id it spotted in another session's scorecard data, instead of resolving its own — the same fabricated-footer failure mode flagged in each of the last two digests, recurring for a third consecutive day.
